Highlights

• when clicking on submit to GMC, the button will disappear while the submit is under way and a "please wait" message will be displayed. Also, on the server side, we will no longer resubmit revalidations that where submitted successfully before. This should fix the "Open recommendation exists" bug.
• revalidation has been setup for HEE North West
• local offices have been added as reference data
• trainee surnames should now properly be pulled in from intrepid when a trainee's name has been changed and the trainee entry with the wrong name has been marked as invalid
