Process Steps
  1. Select
Case management from UI menu
  • Select Create a new referral/case
  • Enter relevant field data for new case and complete Form A - Referral (Form 1 & 2)
  • Add action plan/s to case
  • Record required outcomes
  • Add document/s to case
    1. contact to send case referral
    2. Complete referral form
    3. System auto-saves details as they progress*
    4. User
    saves and submit case
    1. selects to submit a referral
    2. System presents confirmation requesting user confirm they will
      1. accept terms & conditions
      2. send referral
