2024-01-24 GMC Overnight Doctor Sync Connection and Disconnection

The code currently under review for TIS21-5226 would introduce some timing issues, documented below.
These need resolving in order to deliver the value of the ticket.
Timing issue #1
Dr A should remain connected to their DB but will be disconnected
Timing issue #2
Dr A will be disconnected even though they should be in a new DB
