Summary

Unable to save manually entered assessments that require a drop down reason to be selected.

Impact

End users were unable to use the UI to save these type of assessments

Root cause(s)

  • Angular 9 upgrade with Ivy as default (which has a new way of compiling and rendering Angular HTML files to JavaScript)

Trigger

  • End users were unable to save manually entered assessments in the UI that required a drop down option to be selected for ‘Reason’ (Bulk upload was unaffected)

Resolution

  • Refactored the Angular HTML file.

Detection

  • MS Teams - messages from end users not able to save records.

Timeline

  • 08:36 - First MS Teams message from end user

  • 13:30 - Fix pushed to prod
