Revalidation - Recommendation - Detail Create/Edit Screens

The fields relevant to the detail view only screens are detailed below, key points to note are that this screen is:
Editable view
Links out to non-Revalidation areas of TIS should be disabled
Should only be viewed/editable by Admins in the trainee’s local office, or who are associated with a trainee’s past local office
